Oligoclonal Bands (IgG) in Cerebrospinal Fluid (CSF)
Oligoclonal bands (OCBs) are special types of proteins (called immunoglobulin G or IgG) that show up in cerebrospinal fluid (CSF), the fluid that surrounds your brain and spinal cord. These bands form when the immune system produces antibodies in the central nervous system (CNS). Finding these bands in the CSF can help doctors diagnose certain diseases affecting the brain and spinal cord, especially those involving inflammation.
How OCBs Are Detected:
To detect OCBs, a lab test called isoelectric focusing (IEF) is used, which separates the proteins in the CSF based on their electric charge. Doctors compare the proteins found in both the CSF and the blood (serum). If OCBs are present in the CSF but not in the blood, it indicates that the body is producing these antibodies inside the brain or spinal cord, suggesting there is an immune response happening there.
Why OCBs Matter:
Oligoclonal bands are most commonly linked to multiple sclerosis (MS), a disease where the immune system mistakenly attacks the protective covering of nerve fibers in the CNS. About 85-95% of people with MS have OCBs in their CSF, making it an important tool for doctors when diagnosing the disease. However, OCBs can also be found in other conditions, such as:
→ Neuromyelitis optica spectrum disorder (NMOSD), a condition similar to MS
→ Infections of the CNS, like meningitis or encephalitis
→ Autoimmune diseases like lupus, when they affect the brain
→ Neurosarcoidosis, a rare inflammatory disease that affects the nervous system
→ Paraneoplastic syndromes, a group of rare disorders that occur when cancer triggers an immune response affecting the nervous system
→ Chronic inflammatory demyelinating polyneuropathy (CIDP), a disorder that affects the peripheral nerves
Limitations of OCBs:
Even though OCBs are very useful for diagnosing MS, they are not always present, especially in the early stages of the disease. Additionally, OCBs can appear in other conditions, so their presence doesn’t automatically mean someone has MS. Doctors need to look at other factors, such as MRI results and symptoms, before making a diagnosis.
Oligoclonal bands are an important biomarker that help doctors diagnose diseases affecting the brain and spinal cord, especially multiple sclerosis. However, the test must be considered alongside other tests and symptoms to make an accurate diagnosis.
