What total protein measures
Serum contains hundreds of distinct proteins. This test does not distinguish between them; it reports their combined concentration in grams per deciliter.
Two families account for nearly all of it:
- Albumin — a single protein, made in the liver, that holds fluid inside blood vessels and carries hormones, drugs and other substances.
- Globulins — a mixed group including antibodies, transport proteins, enzymes and complement. Mayo notes that plasma proteins are synthesized predominantly in the liver, while immunoglobulins are synthesized by mononuclear cells of the lymph nodes, spleen and bone marrow.
Those two respond to different things and can move in opposite directions at once, leaving the total unchanged while something meaningful happens underneath.
That is the central fact about this test: the sum can hide the signal.
Two mechanisms, one number
Mayo Clinic Laboratories sets out the two general causes of an altered serum total protein: a change in the volume of plasma water, and a change in the concentration of one or more specific proteins.
The distinction has a practical signature. In hemoconcentration and hemodilution, all individual plasma proteins are affected to the same degree — so albumin and globulin shift together. When a specific protein family changes instead, the shift is confined to that fraction.
This is why the components matter more than the total, and why "is my total protein high?" is a less useful question than "which part of it moved?"

Laboratory intervals vary more than people expect. Mayo Clinic Laboratories publishes 6.3 – 7.9 g/dL for patients aged one year and over, and notes that reference values have not been established below 12 months of age. Against that interval, a result of 8.0 is elevated. Against the band used here, it is comfortably normal.
Neither is wrong. Reference intervals are derived by each laboratory from its own methods and population, and Mayo's are Mayo-derived. The interval printed on your own report is the one that applies to your result — which matters most precisely for results in the 6.0–6.3 and 7.9–8.5 zones, where different laboratories genuinely disagree.
If you remember only one thing
An abnormal total protein is a prompt to look at its components, not a finding in its own right. First find albumin, and if reported, calculated globulin and the A/G ratio. If globulin is not shown, it can be derived by the laboratory from total protein minus albumin.

This table establishes which component moved. It does not establish why. Where both components shift together, that is the pattern seen when plasma water changes — but these measurements do not establish hydration status, and other explanations including laboratory variation remain possible.
Note the final row. A normal total protein does not exclude a meaningful change in either component.
Two people, same result
Two people both have total protein of 8.7 g/dL.
Person A has albumin and globulin both marked high by the reporting laboratory. Both components contribute to the elevated total. The panel does not establish whether concentration, laboratory variation, or another cause explains the pattern.
Person B has albumin within range and globulin marked high. The globulin component accounts for the elevation, but total protein cannot identify which globulins are increased or why.
Same total, different component pattern. The result determines where to look next — not the cause, and not the next test automatically.
What this result cannot tell you
- Which protein family changed. That requires albumin and globulin separately.
- Which proteins account for a changed globulin component. That requires additional testing when clinically indicated, such as serum protein electrophoresis, immunofixation, or quantitative immunoglobulins.
- Whether you are dehydrated. Plasma water changes are one recognized mechanism, but this test does not measure hydration.
- Whether a normal total means normal components. Opposing shifts can offset.
- Whether an abnormal result is new. That needs a prior result to compare against.
- Whether a serious cause is excluded. Mayo states that normal total protein levels do not rule out multiple myeloma and other malignant paraproteinemias.
Common interpretation mistakes
- Reading the total without the components. The most consequential error on this marker, and the reason albumin sits next to it on the panel.
- Assuming a normal total means nothing has changed. See the final row of the money table.
- Comparing your result to a range from a different source. The spread between published intervals is wider than most people assume.
- Reading a high result as evidence about diet. A high serum total protein is not interpreted as evidence that you ate too much protein. Dietary intake and circulating albumin-plus-globulin concentration are different measurements.
- Reading a result drawn while lying down against a standard interval. Labcorp notes that total protein and albumin normally decrease by 5% to 10% upon recumbency, as during a hospital stay; Mayo independently documents a lower concentration in recumbent collection.

Clinical pearls
- Posture changes the result. Labcorp states that total protein and albumin normally decrease by 5% to 10% upon recumbency. A sample from a hospital inpatient is not directly comparable to an outpatient draw.
- The components are where interpretation happens. Labcorp states that clinical interpretation is greatly enhanced by examining the fractions composing total protein; Mayo's catalog goes further and states that electrophoresis should be performed to evaluate the cause of an abnormal total.
- Hemolysis invalidates the sample. Both Labcorp and Mayo list hemolysis as a cause for rejection.
- Anticoagulant matters. Labcorp accepts serum or lithium-heparin plasma and specifies that oxalate, EDTA and citrate plasma must not be used. Mayo validates serum only.
- Timing of separation matters. Labcorp specifies separating serum or plasma from cells within four hours; Mayo specifies centrifugation within two hours.
- Age matters at the youngest ages. Mayo notes reference values have not been established for patients under 12 months.
Clinical Takeaway
Total protein is a screening sum, not a specific measurement. Its value lies in flagging that something in the protein compartment has shifted; its limitation is that it cannot say whether a specific protein changed or the plasma water did. Interpretation requires albumin and globulin from the same panel and the reference interval printed on that report. An isolated result marginally outside the band, with components inside their own intervals and no symptoms, requires context before a cause can be assigned.
