Precoproporphyrin III is one of several atypical porphyrins measured to assess disturbances in heme biosynthesis and porphyrin metabolism. Unlike classical porphyrins produced during normal heme synthesis, precoproporphyrins form when the pathway experiences oxidative stress, enzyme inhibition, or altered flow of intermediates. Their appearance may indicate that the typical porphyrin sequence is being bypassed or disrupted.

Scientific literature has documented increases in precoproporphyrins—especially Precoproporphyrin III—in the presence of certain environmental toxicants, most notably high-level mercury exposure. Mercury and related toxicants are thought to affect enzymes such as coproporphyrinogen oxidase (CPOX), contributing to the formation of these atypical metabolites. However, elevations are not exclusive to any single exposure and may also reflect liver stress, impaired detoxification, nutrient insufficiencies, or general metabolic strain.

Precoproporphyrin III is best evaluated as part of the complete precoproporphyrin group (I, II, III, and total precoproporphyrins), as elevations across multiple markers are more informative than changes in a single component. Their interpretation becomes clearer when combined with the main porphyrin panel and clinical context.
