Precoproporphyrin II is a non-classical porphyrin that is not normally produced in significant amounts during standard heme biosynthesis. Its presence suggests that the porphyrin pathway may be experiencing oxidative stress, enzymatic disruption, or an imbalance in the processing of intermediates. Because these molecules are formed outside the usual series of porphyrin reactions, they can serve as indicators of metabolic or environmental burden.

Elevated levels have been described in research examining toxic metal exposure, particularly mercury, which can alter porphyrin enzyme activity and lead to the formation of atypical porphyrin species. However, increases are not specific to any one exposure and may also arise from liver stress, nutrient deficiencies, impaired detoxification capacity, or increased oxidative load. Precoproporphyrin II is best interpreted as part of a pattern across all precoproporphyrins rather than individually.
