Ranges are assay-dependent and genuinely vary between laboratories. On its current platform, Mayo Clinic Laboratories lists 1.3 – 9.6 IU/L for adult men, 1.9 – 14.6 in the follicular phase, 12.2 – 118.0 at mid-cycle, 0.7 – 12.9 in the luteal phase, and 5.3 – 65.4 after menopause. The range printed on your own report is the primary comparison because it belongs to the assay that measured your sample. Units are numerically equivalent whether reported as IU/L or mIU/mL.

The LH-to-FSH ratio is a pattern hint, not a test for PCOS. Current diagnostic criteria use two of three features after excluding other causes: clinical or biochemical hyperandrogenism, ovulatory dysfunction, and polycystic ovarian morphology by ultrasound or, in adults, an elevated AMH used in place of ultrasound. LH and the LH-to-FSH ratio are not among those criteria. Hormone testing is also not routinely needed to identify natural menopause in otherwise healthy people aged 45 or over with typical symptoms; when biochemical confirmation is appropriate, FSH is used rather than LH.
Two people with the same result
A worked hypothetical: two people each receive an LH result of 15 IU/L. The first is a 52-year-old woman whose FSH on the same draw is 70 IU/L and whose periods stopped a year ago; her LH fits expected menopause physiology. The second is a 28-year-old man whose morning testosterone on the same draw is below his lab's range; the identical LH value now suggests the pituitary is increasing its signal while testicular output remains low. Same number, different meaning, separated by the response hormone and life stage.
What this result cannot tell you
A serum LH value cannot confirm or rule out pregnancy; that is hCG's job, and LH is expected to be low in pregnancy. A single LH result also cannot prove that ovulation occurred: urinary LH kits identify a surge that generally precedes ovulation by 1 to 2 days but provide indirect evidence, while appropriately timed luteal progesterone provides evidence of recent ovulation. LH alone cannot distinguish a surge from menopause, primary gonadal dysfunction from a central signaling problem, or medication feedback from disease. Without cycle or life-stage context and the relevant companion hormones, a mildly out-of-range result may remain uninterpretable.
Common interpretation mistakes
The first mistake is using the wrong range: a mid-cycle result judged against a follicular-phase interval can look abnormal when it is not. The second is reading LH without its response hormone; even a "normal" LH may be inappropriately normal when testosterone or estradiol is low. Other errors include treating postmenopausal elevation as disease, overlooking expected suppression from hormonal contraception or testosterone therapy, using the LH-to-FSH ratio as a PCOS test, and comparing values across laboratories as if assay ranges were interchangeable. IU/L and mIU/mL are numerically equivalent, but the reference intervals may differ.

Your doctor may ask where you were in your menstrual cycle on the day of the draw, when your last period occurred, and whether pregnancy is possible. They will want to know about hormonal contraception, testosterone, anabolic steroids, fertility medications, recent illness, major weight change, and training or underfueling. Symptoms help direct the next step: missed periods or hot flashes; low libido or fertility difficulty; headaches, vision changes, or nipple discharge. Bring prior LH, FSH, estradiol, and testosterone results. Ask your testing laboratory about biotin-containing supplements; Mayo requests a 12-hour pause before its assay.

FSH is a frequent companion because it helps separate primary gonadal dysfunction from reduced hypothalamic-pituitary signaling. In men, interpret LH with morning total testosterone, repeating testosterone when low; free testosterone may help when indicated, and prolactin is added when a central pattern is suspected. In women, add estradiol and cycle timing; use hCG when pregnancy is possible and luteal progesterone when the question is whether ovulation recently occurred. Prolactin and TSH are selected according to menstrual history and symptoms rather than ordered automatically for every LH result.
