Lipase is a digestive enzyme made by the pancreas. A blood lipase test measures how much enzyme activity is circulating — a different question from how well your digestion is working.
There is no single universal normal range for it. Published adult upper limits found for this page span 50 to 160 U/L, depending on the laboratory and the assay. A result of 100 U/L is above the upper limit at some laboratories and inside the interval at others.
So the interval printed beside your own result is the only one that applies to it. Comparing your number against a range found online is the error easiest to avoid on this marker.
The figure that carries clinical weight is not the reference interval at all. It is three times the upper limit of normal for the laboratory that ran the test. The revised Atlanta classification and the American College of Gastroenterology guideline both set that multiple as the laboratory criterion for diagnosing acute pancreatitis — one of three criteria, of which two must be met.
An elevated lipase is not a diagnosis. It is one input, read against pain and imaging.
What this number measures
The test measures lipase activity in serum or plasma, in units per litre. It is a catalytic measurement: the laboratory times how fast the enzyme in your sample breaks down a substrate rather than counting molecules.
That is why the ranges disagree. Different laboratories use different assay methods, and the resulting activity values are not interchangeable between platforms — which is why each laboratory sets its own interval for its own method.

Published adult upper limits found for this page span 50 to 160 U/L, so a number clearly abnormal against one is clearly normal against another. The floors disagree about whether a low lipase is even definable: some publish 12–14 U/L, some publish zero, and one publishes none at all.
The Merck Manual states the principle plainly: "A patient's test values should be interpreted based on the reference range of the laboratory in which the test was done; the laboratory typically provides these values with the test result."
Where your result sits
Use the interval printed beside your result. If your report does not show one, ask the laboratory or the clinician who ordered the test; without it your number cannot be placed.
One further point applies to whichever interval you are given. Labcorp publishes different adult bands for men and women and a further shift after age 70, so an adult interval is not necessarily one band.
The three-times rule
The revised Atlanta classification defines the laboratory criterion as "serum lipase activity (or amylase activity) at least three times greater than the upper limit of normal". The ACG guideline uses the same multiple: its 2013 clinical guideline states "serum amylase and/or lipase greater than three times the upper limit of normal", and a 2026 review of the 2024 update in The Hospitalist summarises the criteria as requiring "at least two of three criteria: (1) epigastric or left upper quadrant pain, typically constant with radiation to the back, chest, or flank; (2) serum lipase or amylase greater than three times the upper limit of normal; and (3) characteristic findings on abdominal imaging".
Because the multiple is applied to your own laboratory's upper limit, it moves with your report: 180 U/L where the upper limit is 60, 234 U/L where it is 78, 480 U/L where it is 160. The guideline expresses the threshold as a multiple of the laboratory's own upper limit; that does not make results from different assays interchangeable.
Crossing it supplies one of the two criteria needed. It does not make the diagnosis.

The table establishes which criteria are met. No row of it is a diagnosis.
If you remember only one thing
Find the reference interval on your own report, multiply its upper limit by three, and see which side your result falls on. That one step tells you more than any range you can look up, because it is the step the guidelines specify.
Then read it against symptoms. A lipase above three times the upper limit alongside epigastric or left upper quadrant pain radiating to the back, chest or flank meets two of the three criteria for acute pancreatitis and is urgent. Severe, persistent abdominal pain needs same-day assessment whatever the number says.
Two people, same result
Two people both have a serum lipase of 240 U/L from a laboratory whose upper limit is 78 U/L. Both are above three times that limit, which is 234 U/L.
Person A has constant epigastric pain radiating to the back that began overnight, with nausea and vomiting, and normal kidney function. Pain plus enzyme is two of the three criteria — the presentation the threshold was written for.
Person B has no abdominal pain. The lipase came back on a routine panel and their kidney function is impaired. Reduced clearance from renal impairment is one of the documented non-pancreatitis causes of a level above three times the upper limit.
Same number, different context. Kidney impairment is an alternative explanation, not proof of cause; a result above three times the upper limit still needs clinical assessment.
What this result cannot tell you
How severe a pancreatitis is. This is the misreading with the largest consequences. ARUP Consult states that "the degree of elevation does not correlate with prognosis" and that "serial measurements are not necessary, as they do not provide prognostic information". A Cleveland Clinic Journal of Medicine review agrees: "the evidence for utilizing lipase as a prognostic marker is weak, and far stronger risk-stratification tools exist". A lipase of 2,000 U/L is not five times worse than one of 400 U/L.
Whether the pancreas is the source. Renal impairment, macrolipase formation, hepatobiliary, gastroduodenal, intestinal and neoplastic disease, critical illness, drugs and infections are all documented causes of a lipase above three times the upper limit.
Whether you have pancreatitis. The enzyme is one of three criteria; two must be met.
Whether your digestion is impaired. A blood lipase measures circulating enzyme, not enzyme delivered to the gut.
Anything by comparison with another laboratory's range. Published adult upper limits found for this page span 50 to 160 U/L.
Whether you are recovering. Repeat testing is recommended against once the diagnosis is established.
Common interpretation mistakes
Reading the size of the number as the size of the problem. The degree of elevation does not correlate with prognosis, and the severity scores used in acute pancreatitis — APACHE II, BISAP, Ranson — do not use the lipase value at all.

Treating any elevation as pancreatitis. The guideline threshold is three times the upper limit, and it is one criterion of three.

Assuming a raised lipase points at the pancreas. Reduced clearance in renal impairment and macrolipase formation are established alternatives.
Repeating the test to follow progress. Serial lipase after diagnosis does not gauge progress or prognosis.
Lipase and amylase
Amylase is produced by the salivary glands as well as the pancreas, so elevations occur across a broad range of non-pancreatic conditions and its specificity is limited. ARUP Consult states that "amylase measurement is no longer recommended for the diagnosis of acute pancreatitis" because "lipase offers improved sensitivity and specificity".
Timing separates them too. Serum lipase increases within 4 to 8 hours of the onset of acute pancreatitis and remains elevated for 8 to 14 days, while StatPearls reports that amylase concentrations "often normalize within 3 to 5 days". That is why someone presenting several days into an illness can have a normal amylase and a still-raised lipase. Both use the same three-times multiple.

Clinical pearls
Lipase is measured as enzyme activity, not enzyme mass, which is why intervals travel with the assay. LOINC codes the serum result as 3040-3, property CCnc, catalytic concentration. Two laboratories using different methods can both be right and still print different numbers for one sample.
Not every laboratory publishes a lower limit. UW Medicine reports the interval as an upper limit only, under 70 U/L, while Labcorp publishes floors of 13 U/L for men and 14 U/L for women, Mayo publishes 13 U/L, and the MedlinePlus and Cleveland Clinic figures start at zero. Against an upper-limit-only interval a low lipase cannot be flagged at all.
Labcorp's adult interval is not one band: 13–78 U/L for men 18 and over, 14–72 U/L for women 18 to 70, and 14–85 U/L for women 71 and over, plus separate paediatric bands.
Macrolipasaemia — lipase bound into large complexes the kidney cannot clear — is a documented cause of a persistently raised level above three times the upper limit with no pancreatic disease behind it. Non-pathological pancreatic hyperenzymaemia is on the same published list of alternative diagnoses.

An elevation that sits above the interval but below three times it does not meet the enzyme criterion. It is a result to discuss with the clinician who ordered it, read alongside symptoms, kidney function and any amylase or imaging.
Crossing three times the upper limit does not by itself make a diagnosis either. Acute pancreatitis is established by at least two of three criteria: epigastric or left upper quadrant pain, characteristically constant with radiation to the back, chest, or flank; a lipase or amylase above three times the upper limit; and characteristic findings on abdominal imaging. The enzyme supplies one of them.
Other explanations for a level above that threshold are documented. A systematic review of significant lipase elevations not caused by pancreatitis identified reduced clearance of lipase from renal impairment or macrolipase formation; hepatobiliary, gastroduodenal, intestinal and neoplastic causes; critical illness including neurosurgical pathology; non-pathological pancreatic hyperenzymaemia; and miscellaneous causes including diabetes, drugs and infections. Its authors concluded that clinicians should remain vigilant for an alternative diagnosis to pancreatitis in patients with lipase over three times the upper limit.
The size of the elevation is not a severity score. ARUP Consult states that the degree of elevation does not correlate with prognosis and that serial measurements do not provide prognostic information, and a Cleveland Clinic Journal of Medicine review found the evidence for lipase as a prognostic marker weak and recommended against routine serial testing once the diagnosis is established. The severity scores used in acute pancreatitis do not include the enzyme value.
Timing matters when you read the number. Serum lipase rises within 4 to 8 hours of the onset of acute pancreatitis and stays elevated for 8 to 14 days, so a sample drawn very early or several days late does not represent the peak.

Against an interval that starts at zero or has no floor, a "low" flag cannot occur; against one that starts at 13 or 14 U/L it can. The flag is partly a property of which laboratory ran the test.
A low lipase does not by itself identify a cause. It has, though, been associated with pancreatic disorders, and Cleveland Clinic states the association directly: low levels "may mean that the cells in your pancreas that make lipase aren't making enough of it", something that "can happen if you have a chronic condition that causes long-term damage to the cells in your pancreas", and a low lipase "may be a sign of" cystic fibrosis or chronic pancreatitis. That is an association worth raising, not a diagnosis to be read off the number.
What a serum lipase cannot do is measure how well the pancreas is supplying enzyme to your digestion. It measures circulating enzyme activity, not enzyme delivered to the gut, and it is not a standalone test of exocrine pancreatic function. That question is answered with dedicated tests and with the clinical picture, not by reading a low number on a serum lipase.
Symptoms of poor fat digestion — greasy or foul-smelling stools, weight loss you did not intend, bloating or pain after fatty meals, or signs of fat-soluble vitamin deficiency — are worth raising with your doctor on their own merits, whether or not your serum lipase is flagged low. They are what would prompt the tests that actually answer that question.
A genuinely below-range result is worth raising with the clinician who ordered the test — more so alongside those symptoms or a known pancreatic condition, but the number on its own is enough to ask the question. What it is not is an answer: it neither establishes a pancreatic disorder nor rules one out.
