Escherichia coli, commonly known as E. coli, is a Gram-negative, rod-shaped bacterium typically found in the lower intestine of warm-blooded animals. Most strains of E. coli are harmless and even beneficial, producing vitamin K2 and preventing the establishment of pathogenic bacteria in the intestine. E. coli is often used as a bio-indicator to test environmental samples for fecal contamination, with different strains being host-specific, which helps determine the source of contamination.
Transmission
E. coli is transmitted through a fecal-oral route, either directly or indirectly.
Symptoms and Pathology
There are over 700 antigenic serotypes of E. coli, identified based on O, H, and K antigens. Most humans harbor more than one strain of E. coli simultaneously. While most strains live in the intestines without causing any issues, pathogenic strains can cause various infections in humans, including:
- Urinary Tract Infections (UTIs)
- Neonatal Meningitis
- Intestinal Diseases:
- ETEC (Enterotoxigenic E. coli): Causes diarrhea in infants and travelers.
- EIEC (Enteroinvasive E. coli): Causes dysentery-like diarrhea with fever.
- EPEC (Enteropathogenic E. coli): Causes watery, sometimes bloody, diarrhea, especially in children.
- EHEC (Enterohemorrhagic E. coli): Causes hemorrhagic diarrhea and/or food poisoning, which may develop into hemolytic uremic syndrome (HUS). This includes the invasive O157strain, which accounts for 80% of EHEC serotypes and produces the verotoxin or Shiga toxin.
Strain identification requires molecular techniques that are not readily available in most diagnostic laboratories.
Treatment
Antibiotics are not useful for treating acute diarrheal illnesses caused by E. coli and may actually increase the risk of developing HUS by up to 17-fold. This occurs because antibiotics can damage the bacteria, causing them to release more toxins. Antibiotics should only be used if a patient is septic, and recommendations should be based on sensitivity results. For an herbal alternative, the Freedom, Cleanse, Restore protocol can be used.
